Can I keep my current primary care provider?
No. To prevent redundant or conflicting care, your Atlas Medical provider will serve as the only designated primary care provider. Atlas will collaborate with your previous providers to ensure that we have a complete understanding of your medical history.
How often will I be seen by my Atlas provider?
On average, patients are seen once every 4-6 weeks. Atlas providers coordinate care by managing medications and communicating with family and specialists. They also see their patients for urgent needs.
Does Atlas Medical see patients every time they are in my community?
The decision to see a patient is made by the individual provider at each visit to the community. Even if the patient is stable, the provider may follow up with chronic conditions and medication changes or emphasize active prevention of possible complications at each visit. The provider may also work with caregivers and staff to identify any changes in health conditions.
How can I communicate with my Atlas provider or my patient care coordinator?

After office hours and on weekends, our on-call provider is available to provide telephone triage for our patients.
